* South Suburban College requires all incoming students to provide a social security number on their SSC application. Your social security number is required in order for you to receive financial aid or a 1098T, as well as for the college’s compliance with state and federal reporting requirements. Your SSN will be stored in a single secured location and will not be used for internal college business.

 

Other Forms Submission Procedure
Cooperative/Chargeback Agreement Application Form To expedite the approval process you must provide ALL of the following information:

  • An Application for South Suburban College must be on file with the Admissions Office (Room 2348)
  • A valid Driver’s License or State Id is required as proof of current In-District residency
  • An official or unofficial copy of your High School or High School Equivalency (HSE) Transcript
  • A copy of your placement test scores (an official or unofficial copy of your College Transcript can be used in lieu of placement test scores if prior college courses were completed in Math and English)
  • A copy of the PROGRAM you are seeking to pursue (from their catalog and/or program brochure)
  • ANY COURSES NOT PERTAINING DIRECTLY TO THE PROGRAM, INCLUDING DEVELOPMENTAL COURSES AND GENERAL EDUCATION COURSES, ARE NOT COVERED.

Print and fill out this form and bring it to the Counseling Office, Room 2350.

Student FERPA Release Form The Family Educational Rights and Privacy Act (FERPA) of 1974, is a federal law that establishes the rights of students with regard to education records and ensures students the right to privacy and confidentiality with respect to those records.  This law prohibits SSC from discussing your student record with anyone other than yourself unless written authorization has been requested.
Transcript Request Form South Suburban College has retained National Student Clearinghouse to accept transcript orders over the internet. Official transcripts are issued by the Office of the Registrar. Transcript requests are ordered online and are processed within five business days. Go to the TRANSCRIPTS web page for more details.
Withdrawal Form Before making changes to class schedules for the Fall 2020 semester, all students are encouraged to discuss all possible options with class faculty, financial aid, and a counselor. Every effort should be made to retain enrollment status and successfully complete the fall semester. This includes requesting alternative options to protect the time already invested in the course.

Withdrawal Process

  • Students are required to talk with a SSC counselor and request approval to withdraw from a course. Students can reach a counselor via QLESS
  • Students receiving Financial Aid must also discuss the impact of withdrawing with a Financial Aid representative. Students can reach a Financial Aid representative by sending an email to: sscfinaid@ssc.edu
  • A “W” (Withdrawal) may be recorded on the official record for students wishing to discontinue attendance in a course after the end of the refund period. Please review the details of the SSC withdrawal process at https://ssc2.evo-clients.com/academics/grading-system/#withdrawal/
  • Withdrawing from courses impacts tuition payments and balances due. Student billing is not adjusted upon withdrawal from a class.
